Personal Coaching Agreement

Thank you for allowing Provisioned Ministries to assist with your financial planning. We look forward to serving your needs! To help guide our relationship, we ask that you review the information below and enter your initials in the appropriate boxes to agreement.

To schedule an appointment, please email us at info@provisionedministries.com. We understand that your time and resources are extremely valuable, so we ask that you honor your scheduled appointment time. If you need to reschedule, just let us know in advance. Appointments can only be conducted in a face-to-face setting at this time.
If you are married, both spouses must participate and commit to all scheduled coaching sessions. If your spouse is not available for an appointment, we will reschedule.
We assume the responsibility of understanding and evaluating your current needs, goals, and resources in order to develop a strategy to meet your objectives. However, in order to make your plan work, we expect that you will come prepared to take the steps necessary to succeed, take full responsibility for your plan and its implementation, and contact us when you need additional advice and direction.
You are asked to complete preliminary paperwork to better prepare your coach. We ask that these forms be filled out to the very best of your knowledge and ability prior to your scheduled appointment. You assume all responsibility for providing accurate and complete materials, documentation, and facts upon which our coaching may be based.
If you need to reschedule your session for any reason, please give us at least 24-hour advance notice.
We will keep completely confidential all matters discussed, facts revealed, and the nature and content of all documents you provide to us or we generate. We will reveal such matters only upon receipt of a written authorization and clear, verifiable instructions from you telling us exactly what we may reveal.
Coaching is designed to give you accurate and authoritative information with regard to the subject matter covered. We are not authorized to render legal, accounting, or other professional advice. Since your situation is fact-dependent, you must additionally seek the services of an appropriate licensed legal, accounting, or investment service.

Privacy Terms and Conditions

By agreeing to these terms, you are confirming that this agreement is entered into by and between yourself and Provisioned Ministries for the purpose of preventing the unauthorized disclosure of Confidential Information.

1. "Confidential Information" includes all information or material that has or could have any sort of value in your personal affairs.

2. Provisioned Ministries’ obligations under this agreement do not extend to information that is publicly known at the time of disclosure or subsequently becomes publicly known through no fault of Provisioned Ministries or information that is disclosed by Provisioned Ministries with your prior written approval.

3. Provisioned Ministries will hold and maintain Confidential Information in strictest confidence for your benefit. Provisioned Ministries will carefully restrict access to Confidential Information to employees as is required for properly assisting you with financial expertise and care. Provisioned Ministries shall not publish, copy, or otherwise disclose to others, or permit the use by others for any reason whatsoever of any Confidential Information without your prior written approval. Provisioned Ministries shall return to you any and all records, notes, and other written, printed, or tangible materials in their possession pertaining to Confidential Information immediately if you request it in writing.

4. The nondisclosure provisions of this agreement shall remain in effect indefinitely unless you send Provisioned Ministries written notice releasing them from the agreement.

5. Nothing contained in this agreement shall be deemed to constitute either party a partner, joint venturer, or employee of the other party for any purpose.

6. If a court finds any provision of this agreement invalid or unenforceable, the remainder of this agreement shall be interpreted so as best to effect the intent of the parties.

7. This agreement expresses the complete understanding between you and Provisioned Ministries with respect to the subject matter and may not be amended.

8. The failure to exercise any right provided in this Agreement shall not be a waiver of prior or subsequent rights. This agreement shall be binding on the representatives of Provisioned Ministries and any such assignee of yours.
